Supportive follow-up care after a mental health hospitalization is essential for long-term health and wellness.

Going into the hospital for depression, trauma, eating disorders, substance abuse, self-injury or suicidal thoughts can be overwhelming. That’s why our behavioral health team is available to help you transition from inpatient to outpatient care, and prevent a relapse of symptoms.

Success in treatment requires time and effort.

It's important to recognize the possibility of a crisis and be prepared, while also working to prevent it. People who receive follow-up care after hospitalization are less likely to return to the hospital for additional treatment.

Helping you reach the best possible outcome.

Signing a release form allows the hospital to send information about treatment to your mental health providers and PCP. This is an important step, especially if you’ve been prescribed medications.

Your primary care provider (PCP) is part of your care team. Your PCP can talk with you about the importance of taking your medications regularly. If needed, they can also refer you to a specialist.

Keeping track of your appointment schedule.

After you’ve been discharged from the hospital, our team can help you schedule your outpatient follow-up care. We’ll also send you reminders about your appointments and help answer any questions you may have.
